White Paper: Building a Better Model for Care at Home

Key Benefits of Home-Based Palliative Care for the Elderly Population and People with Serious or Advanced Illnesses

90% of today’s elderly population report that they want to age in place as they live longer, stay active, and have more options that enable them to remain at home. An annual Health at Home study revealed that 72% of patients prefer to recover at home because it is an environment where they are more comfortable and familiar with while being able to set their own schedules, enjoy less noise, have less anxiety, as well as have better nutrition and access to their support networks and transportation.

Home-based palliative care (HBPC) supports a structured approach for those with serious or advanced illnesses who need an added layer of at-home support services. More importantly, HBPC can address social isolation and loneliness by helping members to connect with and visit their friends and family. Recent studies on social isolation and loneliness found that social isolation was associated with approximately 50% increased risk of dementia and loneliness was associated with higher rates of depression, anxiety and suicide. That is why it is important that elders get the care that they need and deserve through support systems like HBPC.

Turn-Key Health, a CareCentrix company, serving health plans, provider organizations and their members who are experiencing a serious or advanced illness, released a white paper, “Building a Better Model for Care at Home,” to demonstrate how home-based palliative care (HBPC) programs for individuals with a serious or advanced illness successfully align treatment decisions with personal goals, preferences and values; enable payors to reduce emergency department visits, hospitalizations and 30-day readmissions; address clinical and non-clinical issues, such as social determinants of health (SDoH); and provide an opportunity to enhance member and caregiver engagement and care coordination.
